Preventing Blindness in Kentucky: Eyeglass Assistance

A new program is in effect to assist individuals with the cost of eyeglasses. The "Prevent Blindness Kentucky" program provides a voucher for one FREE pair of eyeglasses in a twelve month period, to adults and children with income at or below 200% of the poverty level. The person must have a valid eyeglass prescription that is less than 2 years old and not have an eyeglass benefit vision plan. This is an excellent no cost benefit.

Contact Prevent Blindness Kentucky at 1-800-828-1179 or direct at 502-254-4973 and ask to speak to someone about the eyeglass program. There is a form that you are required to complete and return along with a prescription for your eyeglasses. Once approved, they will send you a voucher for your eyeglasses. You may take the voucher to one of the following participating vendors to receive your free pair of eyeglasses:

  • Lenscrafters
  • Sears Optical
  • Target Optical
  • Select Pearl Vision locations (not franchise locations, corporate stores only)
